Before you begin, the questionnaire will take approximately 10-20 minutes to complete. It is meant to be completed as a combined household - only one questionnaire per household is required. Gathering certain documents in advance may be helpful - such as a picture of your photo ID(s) (driver’s license, passport, etc.), copies of statements for any accounts you'd like to transfer and a picture of a void cheque that you'd like to connect your investment accounts too.

A trusted contact person (TCP) is someone other than your spouse whom your advisor and portfolio manager could contact in the event you become mentally incapacitated and unable to make sound financial decisions.

A politically exposed person (“PEP”) is a person, or family member of a person, who has held one of the following offices and/ or positions in/or on behalf of a foreign country or domestically. For this discussion, family members include a spouse or common-law partner; child; mother or father; mother or father of a spouse or common-law partner; and sibling.
A foreign PEP is a person who holds or has held one of the following offices or positions in or on behalf of a foreign state. Examples include:
• head of state or head of government.• member of the executive council of government or member of a legislature.• deputy minister or equivalent rank.• ambassador, or attaché or counsellor of an! ambassador.• military officer with a rank of general or above.• president of a state-owned company or a state-owned bank.• head of a government agency.• judge of a supreme court, constitutional court or other of last resort; or• leader or president of a political party represented in a legislature.
A domestic PEP is a person who holds or has held (within the last 5 years) a specific office or position in or on behalf of the Canadian federal government, a Canadian provincial government, or a Canadian municipal government. Examples include:
• governor General, lieutenant governor or head of government.• member of the Senate or House of Commons or! member of a legislature.• deputy minister or equivalent rank.• ambassador, or attaché or counsellor of an ambassador.• military officer with a rank of general or above.• president of a corporation wholly owned directly by Her Majesty in the right of Canada or a province.• head of a government agency.• judge of an appellate court in a province, the! Federal Court of Appeal or the Supreme Court of Canada.• leader or president of a political party represented in a legislature; or• mayor.
